NO MORE WAITING IN CLEARANCE LINES! NO LONGER WILL YOU PRINT AND BRING IN A PHYSICAL TO THE ATHLETIC DIRECTOR. YAY! The athletic clearance process is all done INDEPENDENTLY, ON YOUR TIME, through Final Forms and completing the steps below.
MORE GREAT NEWS: BASELINE CONCUSSION SCREENING (BCS) ARE NO LONGER REQUIRED PRIOR to participate in contact sports. Any athlete with an expired or nonexistent BCS in a contact sport will receive a concussion screening at practice in the first part of the season FREE of charge scheduled by the PSD.
THE ATHLETIC CLEARANCE PROCESS
EVERY ATHLETE MUST COMPLETE the following CHECKLIST before the Season Starts:
1. Complete Online Clearance using FinalForms
- Follow the link in the email from PSD Parent Square to access FinalForms
- Struggling to find email, try this: https://puyallup-wa.finalforms.com/ or the PSD athletics page.
- Log in or use the "Never Logged In" button to create an account.
2. VALID Sports Physical
- Ensure your athlete has a current sports physical on file in FinalForms
- Through your Final Forms account, if your student has a valid physical on file, you will be able to see your athlete’s physical examination date, and therefore do not need to upload proof of a current physical examination.
- If the dates shown in Final Forms expires during the sport season, your athlete will NOT be cleared.
- If necessary, to obtain a current sports physical, schedule with your primary care provider or a walk-in clinic.
- Physicals can be valid for up to two years from the date of the examination, provided this duration is explicitly noted by the physician on the physical form. Otherwise, it will be valid for only the period the physician specifies.
- Upload the physical form to Final Forms as an image (.pdf/.jpg/.png/etc.) to be reviewed by the AD.
3. Select Sports for your athlete in FinalForms
-Select the sports your athlete plans to participate in for each season.
- Only select one sport per season (i.e. do not select cross country and volleyball)
- If the athlete changes their mind later, adding or deleting a sport, you can always adjust in Final Forms.
4. Complete Parent Forms in FinalForms
- Fill out ALL required online forms in Final Forms.
- This can be done one time for the entire school year. I.E. you can select and sign the forms once for the entire school year.
- Once this is completed, an email will be sent to the athlete’s school email address.
5. Complete STUDENT/ATHLETE forms in FinalForms
- AFTER THE PARENT COMPLETES THEIR FORMS, an email will be sent to the student to login and complete the student forms through their personal Final Forms account.
- Final Forms communicates with students using their district issued email account: first.last@students.puyallupsd.org
- The easiest way to access a student email is to use their district issued device and log into their Outlook account through Microsoft Office.
- If the student does not have access to a district device, PSD student accounts can be accessed by visiting http://portal.office365.com, using their PSD Clever account, OR accessing their PSD Microsoft 365 Office account.
- If the student cannot remember their password or “forgot my password” does not work, they can contact the PSD HelpDesk (helpdesk@puyallupsd.org, 253-841-1301). This should be a last resort.
6. PAYMENT*
- Pay ASB ($30) and Athletic ($50 per sport) fees via InTouch (https://intouch.puyallup.k12.wa.us).
- Cash and Check payments may be made through the school’s office manager, but InTouch saves time!
- If your athlete is trying out for a cut sport (basketball, soccer, volleyball), wait to pay the athletic fees until after the team selection and the final roster is established.

7. Verify Clearance
- Once all these steps are complete, the Athletic Director will review the information and update the status of the athlete.
- Check Final Forms to ensure all items turn "green," indicating your athlete is cleared to participate.
- Watch for emails that may be sent periodically throughout the process to update you with status.
ONGOING DURING THE SCHOOL YEAR
1. Athletic Eligibility (Board Policy 2151R)
- Ensure your athlete maintains a passing grade in at least 5 of 6 classes and a minimum 2.0 GPA to avoid academic ineligibility.
- If on academic probation, be aware that your athlete will miss 20% of the contests at the start of the season.
2. Stay Informed
- Watch for emails from the athletic director regarding the status of your athlete's clearance, team information, messages from coaches, rosters, etc.
- Dates of the Junior High School sports:
o Football/Cross Country/Volleyball- September-October
o Girls Basketball/Boys Wrestling- November-December
o Girls Soccer/Boys Basketball – February-March/Early April
o Boys and Girls Track- April-May/Early June
HOME SCHOOL
Homeschool students are eligible for participation at their resident school. Parents need to register their students as home-based education with the Puyallup SD each year. Here is the page on the Puyallup SD website where the form can be found. We recommend using the online form rather than the PDF as you will receive an email confirmation. https://www.puyallupsd.org/academic-programs/home-school
Once this is completed with the district, please send an email to your school athletic director with the following information requesting creation of a FinalForms account for home school student. An account will be created for the student and an email sent to the parent to begin completing online forms.
- Student Name (First Last)
- Student Email Address (unique to the student, not the same as a parent email)
When the parent logs into FinalForms, registers their student(s) for a sport (one sport per season), and forms are completed, an email will be sent to the student to login and complete their forms.
